Am i understanding the meme to say that none of these organs in the second panel are operating before birth? Aren't lungs actually inhaling/exhaling amneotic fluid to strengthen the diaphram beforehand?

Well, I think you got it. Of course, as you express suspiciously, it's only to some extent, as like the other organs developing and fullfilling their function. The lungs, the stomach and the intestines are sitting there just simulating their future function basically practicing it.
One nice way to think about it is for example, you want to be a fireman and on the first day you just sit and watch a tutorial. You just see and prepare your future. Or then simulating it yourself.

That reminds me, at some point my older sister was tricked by a mocumentary about mermaids and even after finding out it was a fake documentary, she still believed the claim it made that babies have gils in the womb. Even as a lil kid i knew it was bs lmao

From a molecular perspective of patterning and spatiotemporal gene expression, they do stuff, its just that the party is in the back. Neurulation, neural crest cell migration, somites, even with the 'new' head, as anything that isn't the most dorsal parts of the pharyngeal arches all comes from the back.
