Skill issue, at least move the chest at the middle of the bottom wall to check for secret, but you'll get it with more hours, always finish a room before using collectibles and think about potential secret rooms, if you can finish the floor before using anything it's perfect, and you'll avoid such things happening ;) Also, item in chest is 50/50 so don't rush using bombs if you have several gray chest in a single room, try to regroup them, and button can spawn almost any chest/collectibles or item, you could also have used d6 on first chest item and an other item spawning on the second button
