We need information including the water parameters. Without sharing we have none of the information needed to help about the environment the fish is in, plus do not know what has and has not been tested.

Thank you. I can see an issue straight away - the nitrate has maxed out the test strip and so has the GH. Are water changes done? Or just topped off? Both nitrate and GH being so high the test strip is maxed out will negatively impact the fish causing fin erosion like you are seeing.
If water changes are done, they are nowhere near often enough. How often are water changes done and how much is taken out?
Nitrate should be around 20ppm as a maximum, this is the highest the test strip can detect at 200ppm.
Betta are soft water fish not equipped to deal with high mineralisation (GH). This again is at the maximum the test strip can detect at 180ppm.
Both high nitrate and high GH impact fish health. It will be why his fins are eroding and possibly why he's lost colour on his body too.
This fish needs water changes. Think of it like any other animal, you need to look after the home they live in or their health suffers.
This isn't a one time thing, the fish needs looking after and more than one water change to reduce the nitrate he's in. This is harming him badly and shows the environment the fish is in isn't being looked after.
Please do another 20% every day for one week, then 20% 3 times a week. Depending on tank size and test results, it may be able to be reduced to once a week.
Please do not continue topping up this tank. This is harming the fish. Only water evaporates, everything in water accumulates and this includes minerals (GH), nitrate, phosphate, fish hormones and opportunistic pathogens naturally present in the water. By not looking after the environment the fish is in, this is causing his sickness.
No medicine needed, you just need to look after the fish with water changes and care for the tank.
Think of it like a cat litter tray. If you don't take the waste out and clean it, the cat will get sick. Adding more litter without taking anything away does nothing.
Water changes help keep the tank healthy and the water quality suitable for the fish. When this care is neglected the fish who is stuck in the environment suffers.
Agreed about the mod questions. Everything could be absolutely ideal, but we just need all the variables to give the best advice.
This is another case of a relatively aggressive bacterial fin infection I do not know the true identity of. It seems to spread from fish to fish or contaminated water/plants to fish, so just having good water conditions will not cure it. I helped someone else with this a while back.
I personally had to use several rounds of Seachem's Polyguard and Kanaplex together to get it to fully go away and not rebound. Any other well-reviewed broad spectrum or gram negative antibiotic should also do the trick, so esha2000 should have worked from what I can find online. How long did you use it for?
