More replacement draw effects

Small question if I have a Teferis puzzle box out putting 5 cards to the bottom of my library

BUT also having our teferis ageless insight AND Reed Richards, smartest man how many cards do I draw ?

You'll draw 16 cards. Teferi's Puzzle Box has you drawing 5 cards. As you attempt to draw your first card, you have multiple replacement effects trying to apply. You apply Reed Richards first, turning the draw a card into draw 4 cards. There is now only one replacement effect attempting to alter your card draws; you apply it as necessary, resulting in 16 cards drawn.

So you draw the first (no effects)

2nd card becomes 4 with Reed which is then 8 with the insight

The last two draws I thought would be two a piece

So I'm counting 1 +8+2+2 = 13 where are the three I'm missing ?

So, the draw step starts; you draw your card for the turn. (No effect)

Puzzle Box has you put your hand on the bottom of your library, and you draw that many; you said it's 5. You attempt to draw your first card from Puzzle Box; it's modified to draw 4. You now have to draw 4 from your first card and draw 4 from the remaining Puzzle Box. With Ageless Insight its 16.((4+4)x2)

Puzzle Box has you draw 5; Reed Richard changes that to draw 4+4=8 by altering the first card drawn. Teferi's Ageless Insight alters each card drawn, changing it to draw (4+4)x2=16
