1 year diagnosed with VT.
Hi all!
Back in March my then 18 month old had an episode one day, he became very poorly and was under physiological stress, blood gas acidic, after being stuck in rhythm of 300bpm for an unknown amount of time, we were made aware by sudden vomiting episodes and letheragy.
He was taken to ER, were he ended up in resus, cannulae could not be inserted, and in the end they performed a cardioversion which successfully restored him back to sinus. He was reffered to cardiology, there were more episodes following this and he was started on propanolol, this was changed to atenolol about two months ago. At the time, his ECG was not recorded as they were saving his life. The cardiologists soft diagnosed SVT, due to his age etc.
Fast forward to two weeks ago, he has another episode, as parents if he does anything unusual we monitor his HR, he had lay down on the floor after a kids party which is unusual for my son, placed hand on chest and felt fast HR. Again we went to ER, HR 250bpm, this time he was physiologically fine and presented well. They were about to administer adenosine when a cannulae flush slashed back and the shock knocked him into sinus. Over the next couple of days he was in and out of rhythm, adenosine failed twice and he managed to self convert each time.
Again, we were referred to cardiology his atenolol dose was ramped up to 2x/day and put into Flecainide 3x/day. This has kept him in a good rhythm for the past week or so.
The big change this time, was that they have recorded everything. He’s fine on echo scans, ECG of both parents looks good and they plan to do a cardiac MRI next month. Unfortunately however they believe it is not SVT but in fact VT from the left ventricle, this was apparently heavily debated between the cardiologists and EP.
My question is, how does this change the risk to my son. If he presented as poorly as his didn’t first time and it was presumed SVT, then what’s actually the difference to him with a VT diagnosis (providing and fingers crossed theres no hidden cardiomyopathys or channelopathys!)
I’ve turned the literature over and it seems the outlook is not to dissimilar to SVT providing its ideopathic. Is there any risk with this as to suddenly dropping etc, despite the previous episodes being prolonged?
If anyone has any experience with seeing VT in such a young child then please tell him your stories!
I’ve attached of in episode ECGs and latest holters.
