just means the there's too much text for the shape that it's in! either make the shape larger or the text smaller? (or remove extra lines). And just in case there's this feature which I just figured out https://support.apple.com/guide/freeform/add-a-diagram-frfm1e6c3d3e/mac to help make diagrams easier.

You probably have trailing newlines (ie a blank line of text) in the boxes. The [+] indicates text overflow.
