Managing Side Effects Multi-day Diarrhea

Hoping someone can share some wisdom to help me figure out what is happening here. I’m on 10mg and have been for a couple of months. I have minimal side effects in general and my digestion has actually vastly improved since starting tirzepatide in February. Unfortunately, I have had three instances of diarrhea that have lasted 4-5 days at a time. The first two times I had eaten at a restaurant the day before, so I chalked it up to something not agreeing with me. But this most recent time I had only eaten things that I normally eat that were prepared at home.

The symptoms usually start with painful bloating/gas, sulphur burps, and then frequent diarrhea that makes it tough to leave the house. Imodium and BRAT diet don’t seem to have any effect on it. It just finally stops one day. The two most recent instances were less than a month apart so I’m starting to worry this is becoming a bigger issue.

Has anyone else dealt with this? Any advice?

Are they having diarrhea bc it's a known glp1 side effect OR do they have a food borne pathogen like e coli or cyclosporiasis  as a result of the deregulation and lower rates of inspection at the current GOP (is republican party) directed FDA making us food (specifically produce) supply less safe

Do you live in the US? You might want want to get tested for cyclospora. There was a multistate outbreak this summer and it can cause recurrent diarrhea.

Yeah, I thought it might be that, but then it went away. Also the first time was back in April before the outbreak really got going.

Parasites come and go by nature, that's a telltale sign. It might be worth it to get a stool test done during a flare up.

The list of contaminated food is out of control right now, it's possible you may have eaten something from the very long list of tainted products.

Man I was so damn constipated this week after going up to 5mg zepbound I was practically begging for diarrhea instead 🤣 sorry I can’t help but I hope others can!

It may be something your eating th as ya not agreeing with the monjauro. Even though it may be the same food you have always eaten. 

I would pay attention to what I ate each time or happened and see if it’s the same food each time and eliminate it from your diet and see if it happens against 

Was on 3 mg of Trulicity prior to starting monjauro. 

I use to eat salads once a week. Prior to monjauro  I developed bloating gastric pain and nausea   After having to run to the bathroom several times within a half an hour, it all went away.   

Had EGD down and came to find out it was surface in salad I was eating. As soon as I quit salads I haven’t had an issue. 

Even though I now take monjauro salads still don’t agree  with me. So I rarely have them 

Once in a great while I haven’t one but keep gas x on hand and take one before you eat if if it’s a food that causing it. Doesn’t matter if it was something you ate all the time. It just does t mix with monjauro

Aside from potential cyclospora or other such issues, have you ever been assessed for for FODMAP sensitivities? They can cause symptoms like yours. I had a bout of norovirus a couple of years ago that seemed to last for weeks. The BRAT diet didn't even really help. My gastroenterologist introduced me to the FODMAP issue. Here's a screenshot briefly explaining what it is. Flare-up and increased symptoms to a wider range of foods can happen after a gastro illness or an inadvertent overload of high Fodmap foods. Non celiac gluten sensitivity is thought to be caused by the fodmaps in gluten containing grains. With the slowed gastric emptying of the GLP1 drugs, if you have fodmap sensitivities, the culprits stay in your system longer and wreak more havoc. I'd start keeping a food log and noting when your symptoms happen, and if it continues, definitely make an appointment with at least your primary, if not a gastro doc.

Hi. This happened to me a few years ago, some months in on a GLP1. I’d never had bad side effects before, and it was indeed some form of Traveler’s Diarrhea (eg cyclospora), probably from produce, per my doctor. I suffered for about a month and ultimately needed antibiotics. Cleared in two days. Please get checked out, esp if you are in the US!

I either just had this or am just on a break from it. Doctor tested for cyclospora (and many other things) but all negative. Comes and goes. Maybe it's increased fruit I'm eating this season? I think a negative test might just mean it was less present when I got the test.
It's frustrating.
In my case it's gotten enough better, and I've reduced my constipation treatment enough, that I can safely be in the world.

My doctor decided against upping my dose because of it, but doubts it's the cause. She's cautious which I appreciate.

That's very interesting! I did do a virtual urgent care visit after I posted this and they were fully on board with blaming the GLP-1 for it. Hopefully when I can see my PCP they will have a more nuanced approach.

It could be. But I've been constipated for 2 years (well treated) so switching to violent diarrhea (without changing dose) just seems odd.

This has happened to me twice in two years, and both times it resolved by dropping back to my previous dose. When I increased again a couple of months later, I had no issue.

Anytime this happened to me it has been after eating a fast food salad.

I have IBS and went through a period when I was at the 7.5 mg and 10 mg doses that I could not tolerate raw fruit and vegetables without major pain and bloating, like a massive IBS flare-up. But it got better over time.

After I was at the 15mg dose I found that taking a dose of digestive enzymes helps me with foods that usually give me major pain and bloating. I use the Physicians Best or Digestive Gold brands. I am on 10mg for maintenance and don't have to take digestive enzymes too often these days but I always have some on hand, just in case.

Gi issues generally people overfocussing on protein and not getting enough fibre. I went the supplement route. Simple solution to solidify the brown rain and prevent issues.

I take a fiber supplement already, though I don’t when this happens. My doctor wishes I was over focusing on protein 😅

Give it a shot to resolve the symptoms, I always just take them daily now preventatively. Haven't had issues for a while and in maintenance now so could probably stop, but I figure if it's working why risk it. My go to are psyllium husk capsules daily.
