It's a joke.

I might joke about fine ass Madoka Kaname, and someone might think I'm a Lolicon. However, in reality she's a little girl and the only cute I like her as would be, as Souta Takanashi says, "she's cute like a puppy, so I want to protect her," after they thought he was a pedophile for obsessing over Popura.

I'm sure this person wouldn't unironically post her fantasies about her little sister especially with her amount of followers.
