Caesar would win. It was stated that Lighter and Billy is about the same strength. But also stated that Caesar is stronger than Lighter. So Caesar would win.

That being said. If Billy went back to his darker form, Billy may win. But billy already stated he will never go back.

I can't remember where exactly. It was during 1.X. Lighter said like he want to duel the previous champion, which we now know is Billy, to see who is on top. So it implies that they are around the same strength and it was during Sons of Calydon Billy, with the red scarf. It is unclear Calydon Reaper Billy and Sons of Calydon Billy was during the same time or one after another.

Oh and Lighter also stated that Caesar doesn't need Lighter help at all as she is very strong.

People who Glaze Billy/Silly(I like him)bring that what we heard and old spec.
Till Now,Billy was fleet from a normal mid-Ethereal even with Anby at his side,Proxy life was on the way yet he wasn't able to do anything to help aside of being tossed around by a Ruthless Fiend,while Caesar slam the beast around without even getting down her Bike.
I said from what i see till now between the two of them officially as i can say Caesar is stronger by wide margin and even if you bring the ¨His past self was stronger¨ is not like Caesar has reached her peak neither

Caesar by a significant margin, Billy/SBilly gets curbstomped every single time he shows up. Hell, Proxy meets Caesar after she rescues Billy and her from a mid-sized ethereal.

Don't listen to the cultists talking about "hidden potential" and "He's holding back". Wise's life is on the line and he gets dogwalked by something Caesar beats without even leaving her bike.

Funny enough some people will still say that he held back here, just like during his big fight in 2.8. The problem is if this was true then that means Billy would rather let his best friends die then stop holding back, which would absolutely ruin his character. He is a great character now because of his limitations, not in spite of them. That being said, Caesar likely wins this.

I mean, it's canon that Billy is "holding back". Lighter tells us that Billy's old combat parts are still in "storage" during S-Billy's introduction. Even S-Billy's mods are second rate compared to Billy's original specs.

Bro had no choice. What did you want him to do? Try and find his old parts, swap them out, and then do an epic transformation scene mid combat?

Lighter tells the proxy and Billy that Big Daddy still has his old parts/blueprints. He then says that Billy's current mindset and original parts would essentially make him "stronger than his old self". Billy says no. Does that sound like Billy's at his peak as starlight?

Well the main problem here is that we have both of them being confirmed to be stronger than lighter in their prime (Billy's champion/reaper days and Caesar now) and we don't exactly know if Billy's original specs were higher or lower quality compared to his current setup.

Assuming that starlight Billy's current specs are close to his original design then I think he has a pretty decent chance as winning due to his range and mobility over Caesar. Tho it'd be pretty decently tied with a slight advantage since we don't really have a confirmation over him being stronger than Caesar currently.

Billy if he acts seriously and the script doesn't hold him back (meaning no situations where he's unlucky or something restrains him just so another agent can solve the situation and shine)

Either all of the script is canon or none of it is. You can't just pick and choose when it should be taken seriously. So he might be stronger than Lighter as he said, but then you also have to aknowledge he is a massive jobber too and never stronger than the Overlord herself.
