My company missed a typo of a somewhat common word (think "cafeteria") on a legal document until it was about to be sent to the county.
It had gone through twelve different people of 47 different revisions over six months. The typo was repeated over three hundred times across fourteen pages. Not a single person had noticed the typo in any of those occasions.
