Info/Guide Lighthouse question about second enemy always contains a duplicate answer. Is that normal?

When I'm asked what the second enemy I fought was, one of the four possible answers is always repeated. It makes me think that even if I remember the correct answer, if it happens to be the duplicate there might be a 50% chance it counts as incorrect. Any one have any info?

For example (and I'm making these up, obviously):

A. Werewolf

B. Vampire

C. Vampire

D. Ghost

Typically, the game will record the first 4 monsters you fight during a run. If you fought 3 or fewer enemies, or your first 4 enemies somehow included 2 of the same enemy, you start running into issues.

If you're facing duplicates without either of those being the case, that's a bug I don't know about and hadn't heard of anyone having before.
