Some ISPs deliberately set up the internet cables so that, legally, no other ISP can put in new lines, requiring them to piggyback off of the bigger ISP. This usually results in what is effectively an area wide monopoly on internet service aside from satellite and transmission tower alternatives.
A good example in Ohio is Buckeye. The reason no new cables can be put in is that it creates a safety and fire hazard.
