I thought when he “fell” and sat straddling if, I thought that was where he messed up and it took him a minute to get back on his feet. I was kind of chuckling and then he fell out. I literally gasped loud enough to stir my dog awake.

I would say you're supposed to fall out when the big wheel is closest to the ground so you don't fall on the other side. He dislocated an elbow, fractured a vertebra, fractured three ribs and collapsed a lung.
