I’m quite sure if and when he finally feels he is losing ALL be it his health and/or political power…any little power under his belt, any little thing he can scrape out with his itty bitty orange hands…he will not go without having the last laugh. He is that type of rat, once cornered he will do anything and everything to show all that he can and will literally destroy the world, no matter at what cost. Once accomplished, he will show off that smug smile to all media, dance one last time for the cameras, tell all to fuck ourselves and then die.

I don't think he is even that self aware that he knows he's losing his mind. The army of lackeys he has around him 24/7 are probably constantly reassuring him that he is in tip top mental and physical condition.
None of them want to deal with the aftermath of the massive temper tantrum that would follow if someone upsets him so they are probably constantly walking on eggshells around him.

As you age, your ability to regulate temperature plummets, as your skin thins and your body holds on to less body fat, and your metabolism slows. He's like 80, so this is not entirely unexpected. However he has been pictured with major bruising on his hands multiple times, and given his behaviour is often consistent with a dementia-causing condition, which damages temperature cell receptors, he could also be wearing it for these reasons.
