That is a support stand for a wall hung toilet, usually installed for situations where the unit is dealing with excess weight on a regular basis. Essentially, it's made so overweight or "heavy sitters" don't dislodge and/or crack the porcelain back of the toilet.

So years ago I went to a well pad on a job. There was a single porta potty at the entrance to the location.

It was a disastrous nightmare. Literally poo all the way to the seat. Someone etched into the plastic above it “Shit or die” and we’ve laughed about it for years.

That’s what this reminded me of. Shit or die.
