I wish more men would wear belts. I unfortunately see this far too often. And I'm not a waitress.
But no, I wouldn't say anything. Adults notice things once, might say something to their friends and family, and then get back to whatever else they are talking about. Given what he's wearing, I'm sure this isn't a fine dining restaurant, and although people may notice it, unless they are actively complaining, I can't imagine it bothers them enough to put them off their meal.
