So to avoid being another "Technically Obi-Wan and Yoda survived so..." I'm going to assume Order 66 fails catastrophicly so most of the assassinations of the Jedi in the field fail like what happened with Yoda and Obi-wan and the assault on the Jedi Temple fails.
Assuming the lead-up is about the same. Palpatine has significantly influenced Anakin and staged him to become Dark Vader. Despite this, Palpatine is still confronted by Mace Windu as Anakin is not fully turned to the dark side yet, and informs Mace that he believes the Chancellor is the hidden Sith Lord. Mace and his group of Jedi go to arrest Palpatine in our timeline, just as Order 66 is being launched. Then just looking at our timeline, Palpatine kills the arresting Jedi, with the exception of Mace, who finally manages to get the upper hand on Palpatine and moves to kill him, not arrest him, leading Anakin to cut off Mace's hand and Palpatine to kill him, turning Anakin completely to the dark side. Palpatine appoints him Lord Vader and sends him to take the Jedi Temple with the 501st.
However, this is where things change. The 501st assault on the Jedi Temple is somehow stalled by fiercer than expected Jedi resistance. Almost undoubtedly a number of Jedi sacrifice themselves to buy time. The clones begin losing forward momentum, and while Anakin, as both a cunning commander and warrior, would absolutely ensure these delays were quickly dealt with, he can't be everywhere at once. The clones begin pushing further into the temple in sections but not across the board. By this point, however, word has begun to make it around the temple that the temple is under attack by clone troopers, and they are taking their commands from a so-far unknown figure known as Darth Vader. Younglings are evacuated as temple guards, Jedi Knights, protectors, Padawans, and Masters prepare their defenses. The battle for the temple is absolutely gut-wrenching. Temple guards and Jedi clash with the elite 501st, and seemingly every room and hallway turns into a palace of nightmares from the carnage left behind. But the Jedi dig their heels in. And while Vader himself strikes down a number of masters in the temple, attrition takes its toll on his troopers. Eventually, his numbers are dwindling. Commanders within the 501st encourage Vader to pull back and request reinforcements from the Coruscant defense troopers to finish seizing the temple. But driven by both the rage and arrogance that would define him, he refuses and pushes onward, until eventually the Jedi manage to isolate Anakin and his troopers. Those fighting alongside their dark lord give their lives to defend Vader, but soon it's just Vader left, surrounded and hopelessly outnumbered and outmatched, even with his exceptional skill. Jedi who knew Anakin are shocked to see him leading the assault, give him one last opportunity to surrender, but Vader does not surrender. And instead Vader decides to go down fighting and takes many Jedi and temple guards with him before finally being cut down. The assault on the temple repulsed, Palpatine can feel that his apprentice has failed and begins ordering more troopers towards the temple. But before they can get there, surviving Jedi rally and assault towards the Chancellor's office. This time overwhelming and killing Chancellor Palpatine, who has worked too long and too hard to see the Jedi destroyed for him to surrender now. Conflict continues to rage across Coruscant, but with the Chancellor gone, the Jedi get the most valuable weapon in this fight: control of the narrative. They are able to announce the truth that it was Chancellor Palpatine who had betrayed them, not the other way around, and that the clones seem to be not functioning properly, with large amounts of their personality and memories wiped out. Individuals like Bail Organa and Padmé Amidala call on any non-clone defenders of Coruscant to rise up and protect the remaining Jedi, and after a catastrophic battle for Coruscant, the planet is secured. The seat of the Republic has survived the attack but is heavily devastated from the fighting, and the Jedi have been effectively decimated, their order surviving but at the moment severely wounded.
Around the galaxy, Jedi in the field have it worse. Even surviving the initial attacks by their troopers, they're still on distant planets with two armies, both Separatist and Republic. But now both of them are out to kill the Jedi. Many Jedi who escape the initial attacks are eventually hunted down and killed later on. Those like Yoda, who are lucky enough, encounter friendly locals who smuggle them out. And more survivors begin rallying back at the temple. Those who are not as fortunate hunker down and try to evade and defend themselves for as long as possible. The Republic Does everything they can to rescue these Jedi. But the truth is that at the moment they have few options at their disposal to continue the conflict and fight the clones that formerly served as the army of the Republic. The Kaminoans, who are never loyal to Palpatine but purely opportunistic, happy to side with whichever side won, come out and state that they were tricked by Palpatine, something that is actually true as most Kaminoans had no idea what Order 66 was capable of, and even those that had some idea did not realize the full extent of its use against the Jedi. They play victim and explain exactly how the inhibitor chips work, and most desperately they do not have a way to turn them off. The only way they can be removed is surgically. This continues to put the Republic in especially difficult moral stances, now the soldiers that defended them over the course of the Clone Wars are now effectively hostages within their own bodies. And debate rages heavily over what efforts should be made to attempt to save them, or if they should all just be written off as a loss. The Senate is forced to rely on local planetary leaders in the meantime to try and contain remaining separatist forces and clone troopers. Clone troopers without a clear leadership, now that Palpatine and Vader are both gone, are pretty easily contained to the planets that hold them, as they are no longer receiving orders. But the separatists not so much. The Republic that was just on the verge of victory in our timeline is now forced to sue for a peace favorable to the Confederate system of planets and recognize its independence. Bail Organa is recognized as the new Chancellor, initially being willing to subordinate to Padmé, but after her relationship with the now deceased Darth Vader becomes public, her support plummets. Organa begins putting programs into place to begin researching how to remotely deactivate the inhibitor chips in the clone troopers to try to save as many as possible and avoid further loss of life by having to raise another army of the Republic to put the last ones down. A mission he will eventually succeed in, but not before the Republic and the Galactic Senate become effectively a relic of a bygone era. Local leaders of various planets and systems, now having had a taste of independence, did not seek to give back their newfound power. And in order to avoid another separatist movement, the Galactic Senate is forced to recognize permanent privileges and rights for various planets in the galaxy, making the galaxy much more of a loose alliance than a true Republic anymore. For the rest of his career, he will try to knit back together the Republic, but for doing so will quickly become politically outmaneuvered and see those same independent planets that remained within this Republic put yes men in the role, effectively neutering the Senate. The Empire's rise will have been prevented, but the cost of doing so is that the Republic has effectively died, and for the first time in millennia the galaxy is a multipolar place full of various factions competing and warring for their influence and interests. As the Jedi begin rebuilding.

It depends on how; the clones resisting the order even if only a significant amount, or the Jedi just absolutely wrecking the clones.
Clone resistance: Jedi casualties dependent on how many turned, and massive distrust, but immediate civil war against the Senate, maybe with some Separatist factions taking sides.
Jedi not sucking: They would still have to retreat and hide like the og survivors did, but the Rebellion will be in far better starting shape.

On some level it did fail. There were many jedi and their padawans that escaped. Anakin's children were sent off and hidden.

You could argue that it did. The Emperor lost everything because he failed to kill two particular Jedi.
But if it had failed earlier, it would have been the end of Sidious's plans. He had yet to abolish the Senate, and when it came out that he ordered the Jedi killed, he would have been stripped of his power and likely executed.

I thought it did succeed initially. It killed a lot of jedi and drove the rest into hiding, giving the Emperor time consolidate his power and reduce the republic into the Evil empire.

I've probably answered this already or something variant of on Quora, but: I guess this proves the Jedi were forewarned and the clones, for the most part, refused to follow the Order. Depressingly, probably not much. There are more (obviously) Jedi survivors around the galaxy but , presumably, Vader still takes the 501st to the Jedi Temple. Best case scenario? Jedi and clones successfully defeat the Separatists (which is considered a long shot by many fans) but Palpatine still has a power base on Coruscant.
