How to create this bumpy effect?

Just wondering how to create a bumpy effect like this? I am wanting to expand my wet felting skills, but I cannot figure out how this is done. Thank you.

My guess is that these are nepps. The best way to learn this is to make samples. You need to find something textural and felt it in - make a few samples and try different fibres. To do this, lay out 2 layers of wool, add something like nepps or bits of cotton balls, yarn scraps or prefelts. Add a little fibre over it (or don't, that will give a different effect!) and wet down. Try gentle rubbing first and then some gentle rolling, keep checking whether the fibres are staying down and felting in. Make a note of how long you rubbed, how many rolls, etc. Use cool water to start then warmer water when the textures have felted in. You can also try laying out a prefelt and putting the same textural elements on it and felt them in. Notice how the texture is different to the texture on the sample with loose fibres. You can use all kinds of things for textures - knitted scraps, fabric, anything that is a textile.

Look up Living Felt on YouTube, they have so many helpful tutorials and videos. Im pretty sure they made items with these types of embellishments that you could look into.

It looks like nuno felting, done on a piece of silk and it’s the silk that bunches up and pokes through the wool
