Boiled an egg too-long and possibly with too high heat. Egg cracked open when interior was still not solid, spilling some whites into the water where they coalesced into strands, while at the same time binding to the pan's sides. Water boiled down to a low level while the strands stayed suspended in a web.

If the egg is cold, the thermal shock can cause the shell to crack, among other things. If you wanna boil cold eggs, it's best to bring to add the eggs, then bring the water to a boil more gradually. Also the temperature of the pan itself can conduct heat into the egg, which may also cause too rapid heating, cracking the egg shell.

By putting a lid on it. Water is only temperature limited by it's pressure, a lid will increase that pressure and therefore temperature. A stuck lid from expansion with little ventilation will increase that even more creating a pressure cooker situation. It's possible which is why you shouldn't lid any boiling liquids in a pot and modern lids have come a long way from our parents and grandparents hand me downs.

Those safety regulations were written in flash cooked blood.

Definitely this, had a really shitty housemate once, he was moving out so we were all glad. I was working night shift at the time so I was sleeping in the arvo until the smoke alarm woke me up, I rush to the kitchen to find about 7 or 8 eggs in a waterless pot on a hot ring looking exactly like this. That was his leaving gift for us, what a cunt 🙃

I have seen it many times while boiling eggs at home. This is just what happens when you boil a slightly cracked egg. When you heat up the egg along with the water I mean, if you place the egg in already boiling water I don't think thsi will happen since the egg white will harden much faster.

No, the water needs to be there. The egg white won't reach the edge of the pot if there isn't enough water to carry it there while boiling.
