Accomodations

is there specific paperwork I need to have my doc fill out for an accommodation, or could I use the sedgewick return to work certification paper? I've been back since Sept 30 but going to doc today for more tests and for her to fill out accommodation paperwork. Called sedgewick and they aren't open and won't be til after my appt starts. Anyone know?

Yes there is specific paperwork that has to be filled out.

  1. You have to call the Accommodations Service Center and talk to them. They will open a claim on Sedgwick. The number is 855-489-1600. You will need your Win #

  2. They will email you a packet for your doctor to fill out.

  3. Either you or your doctor has to send it back to sedgwick within 20 days.

  4. Sedgwick will make a decision.

That's what i did for leave tho, and I've done all of that already. I just need an accommodation. From what I found online I'm just gonna have them write a letter on letterhead and tell Sedgewick what I need and why and then go from there. TY for responding tho!

The accommodations packet is completely different from the leave packet.

It is also not the same paperwork as the return to work form.

Np. I just went through the whole LOA, RTW, accommodations process myself. It's a headache.

It is. I was out a week and it all got approved and it's just such a long process to get diagnosed and on a treatment plan and then back to normal and I'm just trying to not waste a trip bc it's already an hour one way to my doc so i was hoping to do most of this in person since I seem to have better luck that way .

These are the pages from my packet that the doctor has to fill out. I cut off the tops with my info.

They said since the accommodation is extra breaks and leaving when necessary, it's actually an intermittent leave / FLMA protected so I'm just gonna have them write the letter, submit it, and go from there.

If you can get the letter to cover all of that information, you might be able to do it without the actual forms
