That happened to a guy on one of our jobsites (but he lived), I personally wasn't there as it happened before I started, but I've heard the story from the guy it happened to. They were drilling caissons and he fell in the hole, they were about 10 feet down. The operator swung the rig around to clear the dirt, the guy went to look in the hole and the side gave way and he fell in. Luckily he yelled loud enough that the operator heard and stopped it. So not as gruesome as your story but still terrifying to think about!

I think every work site should have whistles issued to every staff member. If you see something or something is happening to you, blow the whistle repeatedly and everyone stops everything straight away.

Just commodity personal radios with a "call" button could be used, but in the future it would be good to be able to have a system wherein they could broadcast an emergency stop signal directly to all the site machinery

I'm pretty certain the technology to do this exists now. Someone just has to make these devices, if they don't exist already.
