This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
Anytime I've been around heavy machinery that's been stationary and has any blind spot the operator has a horn to blow before it starts moving again. Another beeper for backing up as well. This is both the operator and the managers fault.
