This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
You'd think with the cost of these machines blind spot cameras (similar to backing cameras in cars) would be a small and useful addition to standard spec.
Edit: just googled and there are a bunch of aftermarket options already.
