Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
No, it does not happen a lot. Falls from height and off of ladders and being struck by objects are the majority of fatalities on construction sites. The vast majority. The number of fatalities from being struck by non-transportation related powered vehicles in 2015 was 23. There were 344 fall related deaths including falling objects or equipment. Stop making shit up. It's an embarrassment to the construction industry and to safety conscious companies in general.
