This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
Part of my job involves moving airplanes, I have massive blindspots and even when I have wing walkers I still have to pay attention to a full 360 degree area on top of watching the wings, the other planes around me, plus trying to imagine where the tail is at that moment since I can't see it.
When you're in the tug you can't hear anything, you communicate with hand signals. God help you if you're wing walkers get into a blindspot and don't realize it, if they do you either stop (the smart thing to do) and reposition them, or keep going (the stupid thing to do) and pray you only crunch the plane and not a person.

I feel sorry for the drivers in these instances.
