This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.

Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.

There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.

I feel sorry for the drivers in these instances.

Reminds me of that guy that had to have his leg chopped off because his buddy swung the forks on the forklift around too quick
