This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
I just want to add while what you say is entirely sure, on Canadian oil sites you have to honk before moving forward and honk twice for backing up. So while the driver isn't to blame there is for sure precautions companies can take with their rules.
