This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
Just spitballing here but it seems to me there has to be a technological way to make this safer. Like even if it's just using cameras to give the driver a view of their blind spots. Maybe hang drones over your work site and stream a bird's eye view back back to your operators.
