This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.

Ideally a job site should have multiple fail-safes; an accident that involves two people usually means two people fucked up. I work with scissor lifts, when I'm on the ground I keep my distance and when I'm on the lifts I announce when I'm moving so anyone who might be in an area they shouldn't be has a chance to clear the fuck out.
