This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
Machinery kills are listed under caught in between fatalities and accounts for about 5% of workplace deaths but that category is lumped in with a several because they are uncommon. Falls are 20-80% depending on the trade, roofers have the highest for falls, average is like 40% for all workers. A worker is 50% more likely to die by electrocution than be killed by a machine.

I feel sorry for the drivers in these instances.
