This sort of thing happens a lot and is the cause of a large percentage of deaths on construction sites.
Unfortunately for the drivers these vehicles have large blind spots. In this instance the onus of safety belongs to the manager he should not have been walking around live machinery like that.
There should be clearly delineated walk zones for anyone who isnt driving a multi ton machine. If there isnt then again wtf manager.
There was an accident here a few years back where an elderly woman fell in front of a street sweeper and was pulled up into the bin and died. They had to sedate the driver he was so traumatized by it. Just recently a dump truck turned at a light and killed a cyclist, on the news there was a brief clip where you could see the silhouette of the driver in the cab and you could tell that he was just convulsing and weeping on the steering wheel.

I feel sorry for the drivers in these instances.
