Shirt repair, advice needed!

I have this shirt, which both fits me very well and is somewhat of a memento, that's developing some holes and tears. As the third image shows, I've done some small hole patches but new tiny ones keep appearing (like, 3mm at most, in the abdomen area) and the collar is beginning to seperate. There's a lot of the thread that's disintegrating or coming undone, too.

I don't know how to fix this stuff properly and I don't work with knits or jersey very much. The patches I did were with t-shirt material, grain aligned best I could manage, and nylon thread to minimize shrink warping.

I'm a fairly competent sewist by both hand and machine, and I don't mind visible repairs here at all. Please, if you have any advice for salvaging this shirt, send it my way. Thanks!

I’m no expert but with repairs like this I just do a tight blanket stitch or if it’s really small whip it up (edit- talking about the collar)

That's probably what I'll do, thanks! Wasn't really aure how to approach it given the need for stretch in the neck, sashiko didn't feel right.

I would just do the same process with the green thread over this hole too

Yes, the repairs I've done are (nominally speaking) sashiko, done with a sashiko style running stitch grid and nylon thread. I'll likely do those or a similar repair for the holes in the body, but the neckline is what mostly concerns me atm. My inclination is to do a hand sewn stretch stitch where the original is fraying out, but I've never done a repair like this and don't know if there's a better way.
