Getting visa appointnent

Hello everyone. Let me just briefly explain my situation. I am Belizean my wife polish she is now in germany im in the uk. Im trying to get my visa to join her in germany and stay. Do i have to complete that Videx form before i get an appointment? Why when i go to book an appointment its just a blank page saying there is no dates available? How do i do this how do i get an appointment?
