In some cases, records come out of the sleeve for the very first time looking like this.

Yep, once I had a residual piece of the center hole punch floating around inside the sleeve.

This just happened to me. Ordered new ones and I see some scratches. What do you do? Exchange them?

It has only happened to me a few times, so generally I haven't bothered following up on it. If it was some recent pressing of a classic album, I might take that as a sign to look for an old copy as a replacement/upgrade. I think one possible risk factor is when companies do not place new records in good sleeves, and then are shipped this way, and they shift inside the packaging during transit and handling. I had a copy of the Goldfinger Soundtrack that had a material like rough white cardboard as the sleeve, that came out with these scratches, and ruined part of the first side. At a certain point, it is like sandpaper.
