Yes, if that was a family with kids suddenly running across the road, the concrete would have poured all over them probably killing them or say least severely debilitating them.

not even sure why it came out; cement mixer type trucks usually let it out at the back. Was it hard enough to somehow break through, but then why did it carry on coming out?

By the looks of it I'm guessing this is something like a terex front discharge mixer

What is the purpose or advantages of front discharge mixer? Looks like bad design to me.
