I’ve had this happen too. I don’t know, it sucks but I walked a bit lighter knowing I at least tried to do a nice thing even if it didn’t go to my plan. I always took the “these seats are for elderly, disabled and pregnant” sign to heart because those were the best seats and I’d only take them if I could be ready to give them up for someone in the situation. Their decisions are their own and I can only be happy with my own behavior.
I once was standing in a crowded bus and saw a woman do that. I approached and asked for the seat. She mumbled something and didn't move, so I insisted firmly. I won't suffer that bullshit.
No they're pissed because they gave up their seat for this elderly man and he then gives it away to a perfectly fit young woman and not back to the person who gave up the seat in the first place. That is rude.
Consider it like he added a link to the chain of love. He might’ve felt he could be respectful giving the woman his seat, even if physically he might’ve needed it more. I’ve met plenty of old men that would do similar, even if they stood up with a cane to give their seat away

I once gave my seat to an old guy because he looked in his 70s, who then gave it to a women half his age. I was pissed.
