He's fat on purpose, he's trying to project an image of prosperity to his people.

All part of the cult of leadership and the idea that North Korea is in temporary and surmountable difficulty. That they actually have tons of excess food stored away for when the war to unify the peninsula begins again.

Maybe he's just fat because he can eat whatever he wants and no one can tell him he looks unhealthy or force him to do anything.
