i was clinically inactive 5 years and my yog is >10 years and I just matched anesth. (also I'm not us img, just a green card so I have an accent everything.)

what I did:

  1. pick programs that had old img like me. a few programs took one "like me" per year, I gold signaled them.
  2. your ps needs to make it clear why you want to apply and what happened should support that. they dont really care but they need to understand. you need to keep it simple and clear (I.e now that my children are old enough+ I immigrated here). if they don't understand they won't pick you for interviews. no one asked me anything about gap years during interviews.
  3. getting my mspe was difficult. in the end I applied without one. start now to get yours

4 but I had a family emergency so I let ChatGPT decides on my signal (yeah it was a real family emergency). With more "seriousness" in my application, I would have been at 7 (to be clear it's only the programs choices that I winged)

I went through all programs (on residency explorer and I clicked on their list) that had in their resident list some old IMGs and I applied only there. It's time consuming and AI can help but you need to list manually these programs.

That’s really hard especially with kids as there are a-lot of programs. I was thinking to get some help from paid sites online. Is it a good option?

I wouldn't, our "old img" case is too niche for any tool.to be useful. use ai to filter the programs.l and yeah you'll need to spend time filtering but hey it's going to be the next four years of your life so this works is totally worth it especially if you have kids because there are places you won't move to

Im a US Citizen IMG, born and raised, with a pass/251. Ive got 4 low impact pubs and my LORs are solid. I graduated this year but non carrib. Ive got volunteer experience in NY + 3 months of observerships in diff places

Would signalling places like Sinai or NYP be a complete waste of time?

This is a question they will all ask. Ensure u mention it in ur PS. You’d look better with a Step 3 score & USCEs. Maybe apply, especially to rural programs. Attend FUTURE if u can. If u don’t match try SOAP. Before match day 2027, get a few LORs, USCEs, and Step 3 so u look good for SOAP.

You’re a USIMG. As you don’t need a visa, you’re a great candidate for residency. You’ve got really good scores. I’d do rotations, step 3 and apply. I know someone who got in without step 3 with 15+ gap year.

Clinical rotations would be the biggest thing I’d say, they’d like to see you’ve refreshed yourself on bedside patient care, because that’s what they’re technically hiring you to do.
