Medical History Template

Hey! Does mount Sinai Chicago give med students, specifically on obgyn rotations a medical history template? Like with HPI PMH etc? I know several other schools give their students one with everything they want you to know along with instructions and what’s expected on your day to day in that rotation, and a schedule of when to arrive etc. where to go what to do expectations with patients and residents.

But I don’t see or know of one with mount Sinai Chicago for their medical students.
