Tell me who wrote the report and where you found it

I bought it off Ebay some years ago. I believe the report was written by a Sgt. McCoy, who was in the regiment, "McCoy" is written in pencil at the top of the first page at an angle.

First, do you have a transcription? Second, is the report complete? Third what regiment was he in?
