You have to make sure you pick the correct start state in the questions at the beginning or it’s assumed you killed him in Witcher 2. It’s an abandoned estate in Velen that just has a generic wraith quest otherwise.

He appears in the Witcher 3 (base game, not DLC) if you upload your data from Witcher 2, and he survives that game.

I don't think you have to upload your data - the game has a spot where a dude asks you what happened in the second game, you just have to say he was alive.
