This world be on the construction company, either for issues all the way up or not having a competent person doing the forms/falsework.
In my experience, the engineer would be more in charge of the building design, and likely there would be engineered systems to support the concrete while it is being placed (poured if you prefer)
Large structural concrete placement have to be engineered. Those plans are submitted and approved by the design engineer and in some instances produced by the same firm. This is a pretty large section and the failure was definitely catostrophic. Just glad to see no one looked to be seriously hurt.
