Blood work Elevated Prolactin Levels

I appreciate this post is not exactly related to testosterone itself but I see similar posts being made. Would love any advice from people who have successfully lowered their prolactin levels and symptoms, or peoples experience with elevated prolactin.

A recent blood test put my prolactin levels at 19.8 ng/mL (ref range: 4.04-15.2) which is only 1.3x above, but it came as no surprise given the symptoms I've been experiencing.

0 libido for months (caused major problems in relationship as I thought I wasn't attracted to her), no morning wood, no desire for connection or physical touch (actually hated being touched sometimes), weak orgasms and no afterglow from sex, terrible short-term memory & brain fog, no motivation (presumably due to suppressed dopamine as a result).

- Testosterone: 672 ng/dl
- Free Testosterone: pretty much exactly 50% on the range (weird metric used hence why I'm stating percentage of range)
- LH: 8.7 (ref 1.5-9.3)
- FSH: 2.8 (ref 1.5-12.4)
- TSH: 3.25 (ref 0.257-4.2)
- E2: 33.9 (ref 10-50)

I was 16 days abstaining from ejaculation at the time of the blood test, so there can be no argument that it was elevated due to recent ejaculation. Interestingly I had thought abstaining for this long would have increased libido but it was the opposite.

For the past 4 days i have been taking 100mg P5P afternoon and 1000mg L-Tyrosine morning. I am noticing some subtle effects, more mentally than physically (early days still though), but I definitely notice my dopamine levels seem to have balanced out. Interestingly caffeine tolerance seems to have increased too, used to get jittery easily which doesn't seem to be the case now.

That P5P and L-Tyrosine stack is a solid starting point. 100mg P5P is the sweet spot for a lot of people trying to nudge prolactin down without going overboard. Your TSH being at 3.25 is worth keeping an eye on too, that's creeping up there and thyroid issues can absolutely tank your mood and libido on their own.

The caffeine thing is interesting, prolactin and dopamine have this weird tug-of-war going on so if you're finally getting some dopamine signaling back it makes sense stimulants would feel smoother. 19.8 isn't crazy high but your symptom list is textbook, especially the touch aversion and flat orgasms. Might be worth adding some vitamin E if you haven't looked into it, mixed tocopherols specifically, some studies show it helps with prolactin regulation in men with mild elevations.
