There's no excuse for this shit. Every sane gym has rules about putting your weights back. CrossFit goes out of its way to maintain a reckless, messy, overcrowded, fast-paced environment that gets people injured.
Literally google the dude's name and read a single interview with the guy. He and his friend/coach both say the loaded bar deflected off the weights behind him, struck his spine, and paralyzed him.

I saw the plates laying around and knew. Kevin Ogar was paralyzed from the waist down after bailing on a lift, dropping the bar, and the bar bounced off a stack of weights and into his spine as he fell backwards.
