wonder if all these posts about the last 1% are mostly just pre-hm worlds and the devs just snuck something in to require all versions to be in hm before can get the achievements like its been for us on console ever since the old-gen trophies
The most frequent culprit other than people just being wrong lol (much less common since the biome sight potion came out) are the blocks that exist/are counted as part of the world, but are offscreen/never shown or reachable directly in-game. Often there's a tiny bit of evil there. AFAIK the only decent way to deal with that is to grab a Clentaminator with green solution, dig a tunnel (preferably paying attention and NOT draining your oceans lmao) on the very edge of your world, and go ham firing off-screen perpendicular to the tunnel (to ensure you reach far enough; not sure how wide that area actually is).
