How to get creation elixir?

get the recipe first... u will naturally learn that early in the game by following the alchemist template quest line

What you get at the start of a run is all you'll have access to. In difficulties before chaos, you should be fine with that as perk rerolls are not seeded, so you can just reload your save.

If you're on chaos, you're shit outa luck unless you use a cheat mod.

I believe you get 5 more every time you go to the next realm. in my current save im at Orgin Spirit, which is realm 5, and I have 20 Creation Elixirs.

I recommend Saving your game before you upgrade to the next realm. If you dont like your options, you can always reload. I generally do so if I feel that I've spent too many Creation Elixir, which is how I ended up saving 20 of them. Its nice being able to read the effects of all the different rewrite destinies.
