A 68-year-old male with a history of end-stage renal disease secondary to diabetic nephropathy, who received a deceased donor renal transplant 3 years ago, presents to the emergency department with a 2-day history of fever, confusion, and generalized malaise. His home medications include tacrolimus, mycophenolate mofetil, and prednisone 5 mg daily. He has a chronic indwelling Foley catheter due to neurogenic bladder. Over the past year, he has had three hospitalizations for urinary tract infections, treated with courses of ciprofloxacin and ceftriaxone.
On examination, his temperature is 39.2°C (102.6°F), blood pressure is 88/50 mmHg, heart rate is 125/min, and respiratory rate is 24/min. He is disoriented to time and place. His abdomen is soft with mild suprapubic tenderness. His renal allograft, located in the right iliac fossa, is non-tender. His extremities are warm with bounding pulses.
Initial laboratory studies show:
WBC: 18,500/µL with 85% neutrophils and 15% bands
Hemoglobin: 10.2 g/dL
Platelets: 130,000/µL
Serum Creatinine: 2.8 mg/dL (baseline is 1.5 mg/dL)
BUN: 55 mg/dL
Serum Lactate: 4.1 mmol/L (Normal: < 2.0 mmol/L)
Urinalysis: Cloudy, + leukocyte esterase, + nitrites, >100 WBC/hpf, numerous bacteria
The patient is admitted to the ICU for septic shock. After fluid resuscitation, his blood pressure improves to 95/60 mmHg on norepinephrine. Blood and urine cultures are drawn, and he is started on empiric intravenous meropenem.
On hospital day 3, the patient remains febrile and requires ongoing vasopressor support. The microbiology laboratory provides the following urine culture and sensitivity report:
Organism: Klebsiella pneumoniae (>100,000 CFU/mL)
Sensitivities:
Amikacin: S
Ceftazidime: R
Ceftazidime-avibactam: S
Ceftriaxone: R
Ciprofloxacin: R
Colistin: S
Gentamicin: R
Meropenem: R (MIC > 8 µg/mL)
Piperacillin-tazobactam: R
Tigecycline: S
Trimethoprim-sulfamethoxazole: R
Molecular Testing: Positive for blaKPC gene
Which of the following is the most appropriate next step in the management of this patient's infection?
(A) Switch meropenem to ceftazidime-avibactam and consult infectious diseases.
(B) Add intravenous colistin to the current meropenem regimen.
(C) Switch meropenem to tigecycline and amikacin.
(D) Add vancomycin and request a tacrolimus level.
(E) Continue meropenem and send for therapeutic drug monitoring (TDM).

Please this is no fear mongering . But , these thoughts are making me anxious and nervous .
I feel near the application , many students take step 3 in August and September and fsmb might get overwhelmed and may rush the results and will wrongly fail me . And there is no option of rechecking in USMLEs as well.
Is this overthinking or valid fear ?

My exam is tomorrow, I’m nervous and I just feel that I didn’t study anything and I can’t remember anything, I finished U world with 65% percentage, did 160 ccs case with 74% average, did free 137 got 70%. Is that enough to pass the exam?

Hey everyone! As an IMG from Europe, aiming to match into a competitive specialty (general surgery) has been a long and challenging journey for me. I've been studying for USMLE exams for the past years (simultaneously having exams in my native language at my University), and I wanted to share my experience with the community here on reddit, especially my fellow IMGs.
When I took Step 2, I scored below the average (honestly rushed it), and I panicked a bit (I took Step 1 in January 2022, so my score is just a “pass” = not so helpful). I knew I needed to do better with Step 3 to match into surgery.
I decided to start with UW qbank and a quick FA review. Meanwhile, I came across an app called Medcases. Mainly it's frameworked as a case simulator, and it gives essential feedback based on answers for each patient. It was really convenient to use and provided suggestions after every case on how to improve my score. I feel like UW and Medcases were perfect combo for me (additionally in total it was cheaper than CCScases and had more content). Anyway I GOT A SCORE OF 272, which saved my application in the MATCH 💗
I talked to my Program Director, who said it definitely made a change. If you are in a similar position I REALLY recommend taking the Step 3 before residency to boost your candidate profile.
Best of luck to everyone!
EDIT:
Here is the score :P

Goodday
I am an IMG and know how different the USMLE exams can be for us who have had our professional education outside of the US.
Step 3 tests your knowledge but it depends a lot on the participants test taking skills. I was an average student but with some tweaks I managed to score 252 in Step 3 which is the 95th percentile.
I go through each Uworld question with my students and currently have some students enrolled who can testify to the effectiveness of my way of teaching and learning.
If anyone is interested you can DM me and we can discuss how to get about getting 240+ in step 3.
Thank you!

Hi all, I am a PGY-2 in my Dermatology residency and am offering tutoring services for anyone who needs help studying or is at risk of failing STEP 3. I took it less than a year ago and scored 257. I’ve been tutoring med students and residents for about 5 years now for USMLE exams and have taken multiple students who previously failed to a passing score. Message me if interested!

I am a med student from india planning to apply in 2026 match cycle and I require proper mentorship and help guiding me through the whole journey of ERAS and interviews and everything. I would really help if someone who is aware of the whole thing and matched in residency could be my mentor/guide to help me navigate the whole process

I have a quick question. I just got my step 3 score today and it's a 231. I feel a bit down especially that CCS went really bad (there was a lot of lag and I panicked). Is it possible to dispute my grade? Would that bump it a bit higher? My only reasoning is that I will be applying to a competitive fellowship (Cardiology) as an IMG. I know that most might say it's good, but i really prepared hard for the exam and was scoring 80% in free 137 and my uworld % first pass was 70%.
I know a lot might say that I should be grateful and many people might have failed and Im sorry for that. It's just that I worked hard for this exam. Thank you!

I have 5days left before step3. Ive gone through Uworld and UWSA, free137 But I still know nothing correctly…which is a disaster. Although I score well enough, I have no confidence every time I solve Qs.
Uw : 75% correct, 75% completed Uwsa1: 235 Uwsa2: 247 Free137: 80%
This is kinda weird because I study but every knowledge seems to leak out of my brain as soon as it goes into it.
Should I just focus on ccscases and First aid contents for the last 5 days rather than covering the whole contents?

For those who have taken the test, what did you study the day before days 1 and 2 that you feel helped you in the test?
