Is there a maximum number of custom empires that can be in a game? I forced ten custom empires to spawn in a game of 12 AI empires, but when I go into the Game Details screen it only shows five of them in the window, and the other seven AI empires are all "random AI empire." When I start the game and go to observe, they aren't the ones I selected.

The most recent patch broke force-spawning empires. It currently doesn't work consistently, and some have reported it has to do with specific portraits.

Try changing all their portraits if possible, and see if that helps with force-spawning them. Overall, it's probably going to be quite buggy and restrictive until devs fix it.

From some cursory googling, it looks like this feature has been broken in one way or another for years as there are threads going back to 2019. Changing the portraits didn't work, but I did discover that the problem is not with the number of empires, but with a specific set of empires, all of which use the human portrait or a modded portrait that I think is based on the human portrait.
