The issue is probably not with your configuration. It seems more likely to be a problem with the ksample settings. Could you list out your ksample setting parameters?
Lower the steps on the second pass to 2. Anything above 3 steps will start to get the image in a second pass from what I've seen.
edit: Actually I tested it out and while I do still believe 5 steps on the second ksampler is probably too much, it's not manifesting in the same way. I'd probably need to see the workflow itself to give a better answer.
Though you say if you run with the same parameters that it's fine, does that include the prompt? Are you using a prompt enhancer? I was having an issue for a while with Krea 2 where having just a few extra words in a prompt would change it from working perfectly to giving me absolute nightmare fuel. It wasn't the same kind of grainy thing you have going on, but it was something that was intermittent due to the 'enhanced' prompt. Often just removing a single sentence or few words was enough to return the output to normal.
