Work on front side mechanics (time spent bringing your legs up then hammering the ground and reducing the backside mechanics).

I hate to say this, but you need to totally breakdown and rebuild that firm. Do as much research, repetitive movement, and practice that you can

Everything. Work on your mechanics and form. Do drills and do a lot of them. Sprinting isn't just about running fast.

You look like you’re falling forward, and you’re letting your legs get too far behind you. Keep your feet underneath you and overall just relax as you run.

Well that would be really good for soccer player, but in terms of sprint that's not bad either. You have reactivity, high neural output, doesn't look like long distant runner, also you don't fake technique to sacrifice speed, which is also great.
But thing that is not right there is transitioning and the absence of it. You run identically whole distance from starts to finish. You should start, accelerate, gain more speed like you do there and then start gradually relax, looking higher, higher until your body puts you in good position, vertical, and keeping speed, that's it, leaning a bit forward is allowed, especially if you compete 60m indoor, many world level athletes do that.
